Parish Registration

We extend a warm welcome to all new parishioners and are delighted to have you join our community. To help us get to know you better, please complete the parish registration form. Follow the steps below to download, complete, and return the fillable PDF. Please note the registration form will open in a new window.

Choose How to Complete & Submit

  • Option A: Save to Google Drive (Digital)
  1. Click the Save to Drive icon (the Google Drive logo) in the top right control bar of the PDF viewer.
  2. Open the saved file in Google Drive, fill out the form fields digitally, and save the changes.
  3. Attach the saved PDF to an email and send it to your chosen parish.
  • Option B: Download to Desktop (Digital)
  1. Click the Download icon (the downward arrow) in the top right menu bar of the PDF viewer.
  2. Open the downloaded PDF in your preferred PDF reader (e.g., Adobe Acrobat or your browser).
  3. Type your details into the form fields, save the file, and email it to your parish.
  • Option C: Print & Complete by Hand
  1. Click the Print icon (the printer symbol) in the top right menu bar.
  2. Print out the form, fill in all sections clearly by hand using blue or black ink, and drop it off or mail it directly to your parish office.


Submit Your Completed Form: Once finished—whether completed electronically or by hand—you may return the form in any of the following ways:

  • Email: Attach the completed PDF to an email and send it to the parish of your choice.
  • Mail: Print and mail the form to the parish office.
  • Drop Off: Bring the completed form to the parish office during regular hours.
